Slovak, Czechoslovak, and which is which
Anyone born before 1993 in what is now Slovakia holds documents issued by Czechoslovakia, and the successor registry that reissues them today is Slovak. A single family file routinely contains both, in two closely related languages, and that is normal rather than a problem.
Slovak civil records are rodný list, sobášny list and úmrtný list, issued by the matričný úrad. They carry the rodné číslo, which uses the same structure as the Czech one, including adding 50 to the month for women.
Feminine surname endings
Slovak adds -ová to women's surnames just as Czech does, so a mother, a father and a son appear under two spellings of one family name. We translate them as issued and make the relationship explicit rather than leaving a US reader to decide whether the documents match.

That is expected for anything issued before 1993. We translate them, and the current Slovak reissue if you have both.
Why does my surname end in -ová?
Slovak marks feminine surnames that way. The translation reproduces it and explains it, so it does not read as a different name.
Is the rodné číslo translated?
It is reproduced exactly. It is an identifier, and for women the month carries an added 50, which we note rather than silently correct.
