The občiansky preukaz, and the rodné číslo
The Slovak občiansky preukaz is a travel document within the EU and prints the rodné číslo, the birth number encoding date of birth and sex. The passport does not print it.
So the identity card is the document that carries the identifier Slovak records use, and a US office matching a Slovak passport to a Slovak certificate needs the card or the certificate rather than the passport alone.
The -ová, and the option to drop it
Slovak surnames take -ová in the feminine, so a woman in the Horváth family is Horváthová, and Slovak law allows her to record the form without the ending.
Both appear legitimately, sometimes for the same person at different dates. A US system treating them as different surnames creates a mismatch, and we state that it is one family name.
Hungarian-speaking citizens
Several hundred thousand Slovak citizens are first-language Hungarian speakers concentrated along the southern border, and Hungarian-origin names appear on Slovak documents with Slovak orthography applied.
So a name may appear in a Hungarian form on a family document and a Slovak form on a passport. We reproduce each and note the relationship.

Documents from before 1993 name Czechoslovakia and may have been issued by an authority now in the Czech Republic. They are historical documents named as printed.
