The same birth number, the same +50
A Slovak birth certificate (rodný list) comes from the matrika of the municipality and carries the rodné číslo, which encodes the date of birth with 50 added to the month for women — the convention Slovakia shares with the Czech Republic from their common origin.

Czechoslovak-era certificates
Certificates issued before 1993 name Czechoslovakia and were issued by a registry that is now in one of two countries. That is where the registration happened, not an irregularity, and the translation names the issuing authority as printed rather than modernising it.
Slovak surnames take -ová in the feminine, so a mother and son on one certificate appear under forms that differ by three letters and are one family name. We reproduce both and make the relationship explicit.
What USCIS requires
Under 8 CFR 103.2(b)(3), a foreign-language document must be accompanied by a complete English translation and the translator's certification that it is accurate. Every seal, stamp and marginal note is part of the document, so every one is translated.
