BCG ended in 2012
Slovakia discontinued universal BCG vaccination in 2012, so a Slovak child born after that has no BCG entry and no scar while an older sibling does.

The očkovací preukaz, and compulsory immunisation
Slovak childhood immunisation is compulsory and recorded in an očkovací preukaz, with the definitive record held by the registered general practitioner for children and adolescents.
A certificate can be obtained from that practice. Slovakia uses hexavalent combination products with trade names, and we give the product and the antigens it covers rather than the trade name alone.
The rodné číslo, and the insurer
Slovak medical documents carry the rodné číslo, encoding date of birth and sex, and the health insurer's identifiers. Care is financed through competing insurance companies and the record sits with the treating provider.
We reproduce the number exactly including its slash, and name the insurer and provider on each document.
The -ová, and records before 1993
Slovak surnames take -ová in the feminine, and Slovak law allows a woman to record the form without it, so both appear legitimately.
