The immunisation booklet, and the epidemiologist
Croatian childhood immunisation is compulsory, recorded in an immunisation booklet, and administered through the county public health institute and the paediatric practice, which hold the definitive record.
A certificate can be obtained from the institute even where the booklet is lost. Croatia gives BCG at birth, which is not a US vaccine and leaves a scar examiners ask about.
The OIB, and HZZO
Croatian medical documents carry the OIB, the eleven-digit identification number used for individuals and companies alike, and the HZZO health insurance number.
The OIB does not change with a name and is the cross-check across the file. We reproduce both as labelled.
Where the record sits, and e-Health
Croatian care runs through a registered family doctor who refers onward, and Croatia has built an electronic health record from which a patient can obtain a summary through the e-Građani portal.
Hospitals issue an otpusno pismo — a discharge letter — to the patient and the family doctor. We name each document and its author.
Diacritics, and Yugoslav-era records
Croatian uses č, ć, đ, š and ž, and stripping them changes names. We reproduce the Croatian from the printed page.
Records from before 1991 were made under Yugoslav administration and a modern Croatian document may restate them, naming an authority that no longer exists. We name it as printed. See our Croatian page.
