The surname comes first, and it is usually abbreviated

Telugu names put the house name (intiperu) before the given name, and on documents it is very often reduced to an initial: K. Srinivas Rao is Kondapalli Srinivas Rao. A US form that reads the initial as a first name reverses the person's name and breaks the match against their passport.

Telugu speakers also commonly hold documents from two states — Andhra Pradesh and Telangana — which separated in 2014. A certificate issued before the split names a district that now sits in the other state, and that is not an error. We reproduce the district as printed and note which state issues it today.
