The Mother and Child Protection Card
India records childhood immunisation on the Mother and Child Protection Card issued under the Universal Immunization Programme, and increasingly through the U-WIN digital registry which produces a printable certificate.

Indian schedules include vaccines the US does not use
The Indian programme includes BCG and OPV — oral polio — at birth, and pulse polio campaign doses given outside the routine schedule and recorded separately or not at all.

Indian clinical documentation comes as a discharge summary from an inpatient stay, an OPD card for outpatient visits carried by the patient, and where a case has a legal aspect a medico-legal case report prepared at a government hospital.
They are different documents with different weight and we name each. The physician's registration number with a state medical council appears on formal documents and we reproduce it.
English, and legibility
Indian medical documents are overwhelmingly in English, so a translation is frequently unnecessary and we would rather tell you. What is regularly needed instead is a legible transcription of handwritten notes, prescriptions and OPD cards.
