The immunisation record is centralised
Korea keeps childhood immunisation in a national registry, and a certificate of vaccination can be issued from it at a public health centre or online rather than depending on a paper booklet.
So a Korean applicant can usually obtain a complete printed immunisation history, which is unusual and makes the I-693 straightforward. A paper baby booklet may also exist and both are usable.
BCG, and the two methods
Korea gives BCG at birth by either the intradermal method or the Japanese multi-puncture stamp method, and which was used affects the scar a US examiner sees.
The record states the method, and we translate it rather than recording only "BCG", because an examiner asking about an unfamiliar scar pattern is asking a question the record answers.
The resident registration number on everything
Korean medical documents carry the patient's resident registration number, thirteen digits encoding the date of birth and sex, which ties every document to the person and does not change.
It is the cross-check across a Korean packet. Documents also carry the hospital's institution number and the physician's licence number, and we reproduce all of them.
Certificates, and the romanised name
What a patient receives is a 진단서 (diagnosis certificate), a 소견서 (medical opinion) or a discharge summary, each with a defined purpose — insurance, employment, a court — and the purpose is frequently printed on it.
