The Baby Book, and the barangay health station
Philippine childhood immunisation is recorded in an Early Childhood Care and Development card or a hospital-issued baby book, and doses are commonly given at a barangay health station or rural health unit rather than a hospital.
So the record is a small local card stamped by a health station, and it is what a US civil surgeon needs. The Philippine schedule includes BCG at birth.
Where the clinical record sits
Hospital records are retained by the hospital and the patient receives a medical certificate, a clinical abstract or a discharge summary. PhilHealth claim forms also circulate and contain diagnosis and procedure codes.
A clinical abstract is a summary prepared on request and is the document most often produced for a US purpose. We name each for what it is.
The PRC licence number
Philippine physicians sign with their PRC licence number from the Professional Regulation Commission, and frequently a PTR number for the local professional tax receipt.
The PRC number is how the physician is verified and we reproduce it. Documents for foreign use are commonly notarised, and the notarial acknowledgment page is part of the document.
English, and when you need us
