It has to come from the PSA
Philippine civil records exist in two places: the local civil registrar of the city or municipality, and the Philippine Statistics Authority, which holds the national copy and issues it on security paper. For US immigration purposes the PSA copy is the one that counts — USCIS no longer accepts Philippine marriage certificates issued by local civil registrar offices, and the same preference runs through the rest of the civil documents.
If what you hold is a local registrar copy, order the PSA version before paying anyone to translate it. A perfect translation of the wrong issuing office is still the wrong document.
Late registration, and names
Late registration is common in the Philippines, so a PSA certificate can show a registration date years after the birth, sometimes annotated with the reason. That is ordinary, and the translation reproduces both dates and the annotation exactly rather than tidying them away.
Philippine certificates are largely printed in English already, with Filipino or Spanish-derived terms in places and handwritten entries on older records. Where a document is genuinely all in English, we will say so rather than sell you a translation you do not need. See also our Tagalog page.
What USCIS requires
Under 8 CFR 103.2(b)(3), a foreign-language document must be accompanied by a complete English translation and the translator's certification that it is accurate. Every seal, stamp and marginal note is part of the document, so every one is translated.

A standard translation is for business or personal use where certification is not needed, such as CVs, client intake forms or marketing material, and is billed at the same $24.95 per page of up to 250 words.
