Certified translation of the documents that record medical history in Russia, including the registry annotations and endorsements that carry the legal effect and that a reader unfamiliar with the system will otherwise skip.

The prививочный сертификат

Russia records immunisation in a прививочный сертификат — a vaccination certificate booklet — and in the outpatient card held by the clinic. Entries carry the date, the vaccine, the series and the clinic stamp.

The Russian schedule includes BCG at birth and revaccination, and uses domestic vaccine names and combinations a US reader will not recognise. We give the Russian name, the standard English designation and the date.

The медицинская карта stays at the clinic

The outpatient record (медицинская карта амбулаторного больного) is held by the polyclinic to which the patient is attached, and inpatients receive a выписка — a discharge extract — rather than the inpatient chart.

So what a patient can produce is the vaccination certificate, discharge extracts and certificates (справки). We name each and the institution.

Stamps, and what they do

Russian medical documents rely heavily on stamps: the institution's rectangular stamp, its round seal, the physician's personal stamp with their name and, on some documents, a triangular seal. A document without the round seal is frequently not accepted.

We describe each stamp, translate its text and say where it sits, because a US reader given three impressions cannot tell which one matters.

Handwriting, and Soviet-era records

Russian clinical notes are frequently handwritten in cursive with heavy abbreviation and Latin drug names, and legibility rather than language is the harder problem. Where a passage is genuinely illegible we say so rather than guessing.

Soviet-era records name institutions and republics that no longer exist, and Russian-language records also come from other former Soviet states. We name the issuing state. See our Russian page.
