The książeczka zdrowia dziecka
Polish childhood immunisation is recorded in the książeczka zdrowia dziecka — the child health booklet — and in a karta uodpornienia, the immunisation card held by the clinic where the child is registered.
The clinic's card is the definitive record, so a certificate can be obtained even where the booklet is lost. For an I-693 that certificate is what a US civil surgeon needs, and it is not something a hospital holds.
The PESEL, and the NFZ
Polish medical documents carry the PESEL, the eleven-digit national number encoding date of birth and a sex digit, which does not change and is the identifier every Polish health record uses.
Care is financed through the NFZ, the national health fund, and documents carry its identifiers alongside. We reproduce both exactly, including a leading zero the PESEL frequently has.
Compulsory vaccinations, and BCG
Poland operates a compulsory childhood immunisation programme, and BCG is given at birth — leaving a scar a US examiner may ask about, and it is not a US vaccine at all.
Polish vaccine entries use product names and abbreviations. We give the entry as written and the standard English designation, without mapping one schedule onto another.
Wypis, skierowanie and the doctor's number
A wypis ze szpitala is a discharge summary, a skierowanie is a referral, and a zaświadczenie lekarskie is a certificate for an employer or an authority. They are different documents and we name each.
The physician signs with their chamber registration number (numer prawa wykonywania zawodu), which is how they are verified. See our Polish page.
