e-Nabız gives the patient the whole history
Turkey operates e-Nabız, a national personal health record system in which a patient can see and download their own consultations, prescriptions, laboratory results, imaging and immunisations across public and private providers.
That is unusual and it makes a Turkish file unusually complete. For an I-693 the immunisation section can be exported directly, and a patient told to hunt for a paper card frequently does not know the export exists.
The aşı kartı, and the schedule
Childhood immunisation is also recorded on an aşı kartı stamped by the family health centre, and Turkey gives BCG and hepatitis B early — neither routine in the United States.

The T.C. Kimlik No, on everything
Every Turkish medical document carries the T.C. Kimlik No, eleven digits, unique for life, which does not change with a name and is the identifier every Turkish system uses.
It is the cross-check across the file. Documents also carry the institution code and the physician's diploma tescil registration, and we reproduce all of them.
Heyet raporu, and the dotted i
A sağlık kurulu raporu — a health board report — is issued by a board of specialists at an authorised hospital and carries far more weight than a single physician's note. It is signed by each member and the chief physician with the hospital seal.
