Certified translation of the documents that record medical history in Ukraine, including the registry annotations and endorsements that carry the legal effect and that a reader unfamiliar with the system will otherwise skip.

Form 063 is the immunisation record

Ukrainian childhood immunisation is recorded on form 063 — the certificate of preventive vaccinations — held by the clinic where the child is registered, alongside the outpatient card, form 025.

For an I-693 form 063 is the document, and it can be issued as a certified extract from the clinic. Applicants frequently send an outpatient card instead, which holds the history but is far harder to read.

BCG, and the domestic vaccine names

Ukraine gives BCG at birth, which leaves a scar a US examiner asks about and is not a US vaccine. Entries use manufacturer and product names, some of them regional, rather than generic descriptions.

We give the entry as written and the standard English designation, without inferring a series the record does not state.

Wartime records, and reconstruction

Since 2022 many Ukrainians hold medical records reissued from central systems where local clinics became inaccessible, obtained through eHealth, or created abroad in the countries where they took refuge.

All are lawful and all read differently from a peacetime original. We name the issuing institution exactly and describe the document as the reissue it says it is.

Stamps, handwriting and the patronymic

Ukrainian medical documents rely on stamps — the institution's rectangular stamp, its round seal, the physician's personal stamp — and a document without the round seal is frequently not accepted. We describe each and translate its text.

Names carry a patronymic from the father's given name, which is not a middle name, and clinical notes are frequently handwritten in cursive with heavy abbreviation. Where a passage is genuinely illegible we say so. See our Ukrainian page.
