The vaccination card, and the school requirement
Egyptian childhood immunisation is recorded on a vaccination card stamped at the health office or maternal and child health unit that gave each dose, and proof is required for school enrolment, so families keep it.
Egypt gives BCG at birth. The card is what an I-693 needs, and it is a small document that applicants routinely omit in favour of hospital paperwork.

Egyptian medical documents carry the national number — fourteen digits encoding the century of birth, the date, the governorate of registration, a serial with a sex digit and a check digit.

Egyptian physicians sign with their syndicate registration, and documents carry institutional stamps in Arabic that are part of the authentication. We translate the stamp text rather than describing the page as stamped.
