The Buku KIA
Indonesian childhood immunisation is recorded in the Buku KIA — the maternal and child health book, a pink booklet covering pregnancy, birth, growth and every vaccination — issued at the posyandu or puskesmas.
It is the immunisation record for an I-693 and it is a booklet families keep. Applicants routinely send hospital paperwork instead, which will not have the childhood schedule.
BCG, and the Indonesian schedule
Indonesia gives BCG and hepatitis B early, neither routine in the United States, and uses combination products with Indonesian names.
We give the entry as recorded and the standard English designation. Indonesia also runs national immunisation months delivering catch-up doses, which may be recorded separately.
Where the record sits, and BPJS
Care runs through puskesmas community health centres, public and private hospitals, and the BPJS Kesehatan national insurance scheme, and the record sits with the treating facility.

The NIK, and single names
Indonesian medical documents carry the NIK, the sixteen-digit population number encoding the region of registration and the date of birth with an offset for women, and it does not change.
