The sổ tiêm chủng
Vietnamese childhood immunisation is recorded in a sổ tiêm chủng — a vaccination booklet — stamped at the commune health station or the vaccination centre that gave each dose, and in a national electronic registry from which a certificate can be printed.
The booklet is what an I-693 needs. Vietnam gives BCG and hepatitis B at birth, neither routine in the United States.
The expanded programme, and paid vaccines
Vietnam distinguishes vaccines given free under the expanded programme from those bought privately at vaccination service centres, and the booklet records both, sometimes in separate sections.
So a vaccine a US schedule expects may simply not have been given, or may have been given privately and recorded elsewhere. We reproduce every entry and note which section it appears in.
Where the clinical record sits
Care runs through commune health stations, district and provincial hospitals and a growing private sector, and the record sits with the treating facility. Patients hold a sổ khám bệnh — an outpatient book — and discharge summaries.
The BHYT health insurance card number appears on documents alongside the CCCD. We reproduce both.
Diacritics, and the names
Vietnamese marks tone and vowel quality with diacritics and stripping them changes the word: Hà, Hạ and Há are different names. We reproduce the Vietnamese from the printed page.
