The integrated services card
Egypt's Law 10 of 2018 on the rights of persons with disabilities introduced an integrated services card (bitaqat al-khadamat al-mutakamila), which replaced the older disability card and is the document that opens entitlements.
It records a category and degree assessed by a medical committee, and it is an entitlement card rather than a clinical record. Where a US office needs the diagnosis, the medical report is the document to send alongside it.
The committee, not a physician
Assessment is by a medical committee under the health ministry rather than by a treating physician, and the decision names the committee and its members.
A private physician's report is not a disability determination in Egypt however detailed. We translate each document as what it says it is.
What the card opens
The card carries entitlements to educational support, employment quota placement, transport and vehicle concessions, and exemptions, and the entitlements attached differ by category and degree.

The national number, and the Arabic
Egyptian documents carry the fourteen-digit national number, encoding the century and date of birth and the governorate of registration, which identifies the person and does not change.
