The carné de control del niño sano
Chilean childhood immunisation is recorded in the carné de control del niño sano and in the national immunisation registry, and given through the PNI, the national immunisation programme, at primary care centres.
A certificate can be obtained from the registry even where the card is lost, and it is what an I-693 needs. Chile gives BCG at birth, which is not a US vaccine and leaves a scar examiners ask about.
Fonasa and Isapre, and where the record sits
Chileans are covered by Fonasa, the public insurer, or by a private Isapre, and the clinical record sits with the provider that treated the patient rather than with the insurer.
So a Chilean history is split across providers. We name the institution on each document rather than presenting the set as one continuous chart.
The RUN, and the check digit
Chilean medical documents carry the RUN — the national identity number, the same as the RUT for an individual — written with a check digit after a hyphen that may be the letter K.
It does not change with a name and it is the cross-check across the file. We reproduce it exactly including that character, because a transcription without it fails verification.
The physician's registration, and the epicrisis
Chilean physicians sign with their RUN and professional registration, and the epicrisis — the discharge summary — is the principal document a patient receives from a hospital stay.
