The huisarts holds the record
Dutch healthcare runs through the huisarts, the general practitioner, who is the gatekeeper to specialist care and who holds the patient's central record. Hospitals write back to the huisarts rather than the patient.
So "send us your medical records" in the Netherlands means asking the huisarts for a medisch dossier, which a patient is entitled to. Asking a hospital produces only the episodes it treated.
The Rijksvaccinatieprogramma, and where the record lives
Childhood immunisation runs through the Rijksvaccinatieprogramma, delivered at consultatiebureaus and the GGD, and recorded centrally as well as in a personal booklet.
A vaccination overview can be requested from the RIVM or the GGD, which is what an I-693 needs. The Netherlands does not use BCG routinely, so a Dutch record will not show it and that is not a gap.
The BSN, and the DigiD
Dutch medical documents carry the BSN, the citizen service number, which does not change and is the identifier every Dutch health record uses. Patients access records online through DigiD.
We reproduce the BSN exactly. Dutch documents are heavily abbreviated and the abbreviations are Dutch rather than Latin, so we expand them rather than reproducing initials.
Verwijsbrief, ontslagbrief and the specialist letter
A verwijsbrief is a referral from the huisarts, an ontslagbrief a hospital discharge letter, and a specialist's letter back to the huisarts is the main clinical document in Dutch practice.
They are letters between doctors rather than reports to the patient, which is why they read as terse and why the patient may never have seen them. We name each and its author. See our Dutch page.
