There is no Dutch disability certificate
The Netherlands issues no general disability certificate. There is no card stating a percentage or a grade that a person carries as proof of disability, and a Dutch client asked for one has nothing to send.
What exists instead is a set of purpose-specific assessments: an incapacity-for-work decision, a long-term care indication, a disabled parking card, a public transport companion card. Each is issued by a different body for a different entitlement.
The UWV incapacity assessment
The UWV assesses incapacity for work under the WIA, or the Wajong for people disabled young, through an insurance physician and a labour expert, producing a percentage of loss of earning capacity — not a percentage of disability.
Those are different concepts. A person may be severely impaired and assessed at a low loss of earning capacity, or the reverse. We name the scheme and the concept rather than letting a percentage stand unexplained.
The Wlz indication, for care
Long-term care entitlement runs through a Wlz indication issued by the CIZ, which states a care profile rather than a severity score.
A care profile is a description of the support required and it does not convert into any US measure. We translate the profile as written.
The parking card, and what people actually hold
The document a Dutch person most often holds is a gehandicaptenparkeerkaart, the EU-format disabled parking card issued by the municipality, or a public transport companion card.
Those prove an entitlement rather than a diagnosis, and a US office reading one as a disability certificate is reading too much into it. We name the document. See our Dutch page.
