The libretto, and the ASL that holds the record
Italian childhood immunisation is recorded in a libretto delle vaccinazioni or the pediatric health booklet, and administered by the ASL, the local health authority, which also holds the definitive record.
So a certificate of vaccinations can be requested from the ASL of residence even where the booklet is lost, and that certificate is what an I-693 needs. Applicants routinely ask a hospital, which will not have it.
Ten vaccines became mandatory in 2017
The 2017 law made ten vaccinations compulsory for school enrolment in Italy — including polio, diphtheria, tetanus, hepatitis B, pertussis, Haemophilus influenzae B, measles, mumps, rubella and varicella.
So an Italian child of school age has a documented record of those, and a gap in an Italian booklet is more likely a missing page than a missing dose. We reproduce every entry with its date and the ASL stamp.
The tessera sanitaria, and the codice fiscale
Every Italian holds a tessera sanitaria — the health card, which is also the codice fiscale card — and the codice fiscale, derived from name, date and place of birth and sex, is the identifier every Italian medical record uses.
It appears on prescriptions, reports and discharge letters, and we reproduce it exactly. It is the cross-check across an Italian file.
Cartella clinica, referto and lettera di dimissione
The cartella clinica is the hospital chart and is retained by the hospital; a patient receives a lettera di dimissione on discharge and a referto for each examination or laboratory result.
They are different documents with different content and we name each. A patient may request a copy of the cartella clinica from the hospital records office, which many do not know. See our Italian page.
