The PWD ID, and what it is for
The Philippines issues a PWD ID through the city or municipal office, registered with the local government unit and the National Council on Disability Affairs, under RA 7277 as amended.
Its principal function is commercial: RA 10754 grants holders a 20% discount and VAT exemption on medicines, medical services, transport, restaurants and other goods. It is an entitlement card rather than a clinical assessment, and it is issued locally with a local number.
A medical certificate underlies it
The application rests on a medical certificate from a licensed physician stating the disability, and for some categories an assessment from a specialist. The physician's PRC licence number appears on it and is how they are verified.
The ID itself records a disability type from a defined list rather than a diagnosis, so where a US office needs the clinical finding the underlying certificate is the document to send.
Issued by the LGU, so it varies
Because the ID is issued by each city or municipality, the card design, the numbering and the validity period vary, and a card from Quezon City looks different from one from Cebu.

Validity and renewal
PWD IDs carry a validity period — commonly a few years — and are renewed with the local office. An expired card is not evidence of current status, and the expiry is on the face of the card.
