The tarjeta de vacunación
The Dominican Republic records childhood immunisation on a tarjeta de vacunación issued by the health service, with each dose dated and stamped by the vaccinating centre.
The schedule includes BCG at birth. Cards are frequently handwritten and worn, and where an entry is genuinely illegible we say so rather than guessing at a date, because a guessed date can make a dose look invalid for age.
SNS, SeNaSa and private clinics
Care is delivered through the Servicio Nacional de Salud network, through ARS insurers including the public SeNaSa, and through private clinics, and each keeps its own record.

The exequátur, and how a doctor is identified
Dominican physicians practise under an exequátur — a presidential authorisation to practise a licensed profession — and the number appears in the signature block alongside the Colegio Médico Dominicano registration.
The exequátur is unfamiliar to US readers and looks like a court term. It is the practice licence and we say so.
Medical tourism, and records from private hospitals
The Dominican Republic has a substantial medical tourism sector, and a US patient may hold records from a private hospital in Santo Domingo or Punta Cana in Spanish with English fragments.
