The carné de vacunación and the electronic registry
Peru records childhood immunisation on a carné de atención integral de salud del niño with a vaccination section, and through the national immunisation registry from which a certificate can be printed.
The Peruvian schedule includes BCG and hepatitis B at birth and yellow fever in defined regions. We reproduce every entry with its date, lot and the establishment that gave it.
MINSA, EsSalud, and the split record
Peruvian healthcare runs through MINSA public establishments, EsSalud for contributory insurance, the armed forces and police services, and private clinics. Each keeps its own record.
A person's history is therefore split, and a document from one says nothing about the others. We name the establishment and its code on each document.
Colegiatura, and the CMP number
Peruvian physicians sign with their CMP number — the Colegio Médico del Perú registration — and specialists add an RNE registration. The signature block shows both.
We reproduce them exactly. The establishment also has its own RENAES code, which appears on stamps and identifies the facility.
Certificado médico, and the legal forms
Peru uses defined certificate forms for defined purposes — a general certificado médico, a certificado médico legal from the Instituto de Medicina Legal used to evidence injuries, and forms for disability and occupational health.
