The carné de vacunación and PAIWEB
Colombia records childhood immunisation on a carné de vacunación and, since the national nominal registry, in PAIWEB, from which a printed certificate can be obtained.
The Colombian schedule includes BCG at birth and yellow fever, and in some areas additional doses. We reproduce every entry with its date, lot and the vaccinating institution.
EPS, IPS, and where the record sits
Colombian healthcare runs through EPS insurers contracting with IPS providers, and the clinical record sits with the IPS that treated the patient. Changing EPS does not move the record.
So a Colombian history is split across providers and a patient asking their EPS for records is asking the wrong body. We name the IPS on each document.
The registro médico
Colombian physicians sign with a registro médico number and, since the professional registry reform, a ReTHUS entry. The signature block shows the number and the issuing authority.
We reproduce both. A US office verifying a Colombian physician needs the identifier that matches the register it is searching.
Historia clínica, and the right to a copy
Colombian law treats the historia clínica as confidential and gives the patient a right of access, with defined retention periods. A patient can generally obtain a copy or a resumen de historia clínica.
