The birth number encodes more than you expect
A Czech rodný list carries the rodné číslo, a ten-digit birth number whose first six digits are the date of birth as YYMMDD — except that for women 50 is added to the month, so a woman born in April shows 54 where a man shows 04. A reader who does not know that sees an impossible date and questions the document.
We reproduce the number exactly, digit for digit, and note what the structure means where it helps. Nothing is re-formatted: it is an identifier, not a date.
Women's surnames take an extra ending
Czech adds -ová to most women's surnames, so the son of Mr Novák is Novák and his sister and mother are Nováková. On a US form that reads as two different families, and it is a routine reason for a request for more evidence on a file that is perfectly consistent in Czech.
We translate the names as the documents give them and flag the relationship explicitly, so the difference reads as the grammar it is rather than as a discrepancy. The same applies to older documents issued in Czechoslovakia before 1993.

The month in my birth number looks wrong.
For women, 50 is added to the month, so 54 means April. It is reproduced exactly as printed and explained where that helps the reader.
Why does my mother's surname end differently from my father's?
Czech marks feminine surnames with -ová. It is grammar, not a different family, and the translation makes that clear.
