The občanský průkaz travels in the EU
The Czech občanský průkaz — the identity card — is a travel document within the EU and for a defined set of other countries, so a Czech client may present a card rather than a cestovní pas.
The card prints the rodné číslo, the birth number encoding date of birth and sex with the month increased by fifty for women. The passport does not print it. They are different documents and we name which you sent.
The -ová on the passport
Czech surnames take a feminine ending, usually -ová, so a woman in the Novák family appears as Nováková on her passport. Since a 2022 amendment any woman may instead choose the masculine form.
So a mother and daughter may appear under Nováková and Novák legitimately, and an older and a newer passport for one woman may differ by three letters. We state that it is one surname.
Diacritics, and the machine-readable line
Czech uses á, č, ď, é, ě, í, ň, ó, ř, š, ť, ú, ů, ý and ž, and the machine-readable zone strips them — Dvořák becomes DVORAK.
The printed page and the machine-readable line therefore disagree on the same document. That is standard transliteration and we note it rather than leaving a reader to spot two spellings.
Records before 1993
Documents from before 1993 name Czechoslovakia and may have been issued by an authority now in Slovakia. They are historical documents and the registering authority is named as printed.

Within the EU and some other countries, yes. It prints the rodné číslo, which the passport does not.
Why does the daughter's surname differ?
Czech surnames take -ová in the feminine, and since 2022 a woman may choose the masculine form instead. One surname.
