The immunisation card, and the health post
Nepali childhood immunisation is recorded on an immunisation card stamped at the health post or by a Female Community Health Volunteer, under the national programme, and increasingly in an electronic registry.
So the record comes from a village health post rather than a hospital, and it is what an I-693 needs. Nepal gives BCG at birth, which is not a US vaccine.
Bikram Sambat, converted against a table
Nepali documents are dated in Bikram Sambat, roughly 56 years and 8 months ahead of the Gregorian calendar, with months of varying length that change from year to year.
There is no formula. We convert each date against a table and show both, which matters because a wrong interval between doses can make one look invalid for age.
Where the clinical record sits
Care runs through health posts, district hospitals, teaching hospitals and a large private sector, and the record sits with the treating facility. Patients hold discharge summaries, prescriptions and investigation reports.
Physicians sign with their NMC registration number, which is how they are verified, and we reproduce it.
Devanagari, and the Bhutanese Nepali community
Nepali is written in Devanagari and romanises inconsistently, so a patient name appears several ways. We follow the passport where one exists and note variants.
A large Nepali-speaking population in the United States are Lhotshampa, Bhutanese of Nepali origin resettled from camps in eastern Nepal, and their medical records come from those camps and from IOM. We name the issuing body. See our Nepali page.
