Puerto Rico is a US jurisdiction. Medical records created there are US medical records, governed by HIPAA, requested the same way as records from any state, and there is no apostille and no consular legalisation involved.
An office treating a Puerto Rican record as a foreign document has misunderstood where Puerto Rico is. Saying that plainly saves a step and sometimes a fee, and we would rather say it than charge for treating it as foreign.

Two surnames, and the licence number
Puerto Ricans carry paternal then maternal surnames in the Spanish pattern, and mainland systems index the last word — the mother's name. We keep both and make the order explicit.
