The health house network
Iran built a rural primary care network of health houses (khaneh behdasht) staffed by trained community health workers, and childhood immunisation is delivered and recorded through it and through urban health centres.
So an Iranian immunisation record comes from a village health house or a district health centre rather than a hospital, and the card is stamped by it. That is what an I-693 needs.
The vaccination card, and BCG
Iran gives BCG at birth and runs a compulsory schedule with high recorded coverage, so an Iranian card is usually complete and dense.
Entries use Persian product names and some domestically produced vaccines. We give the entry as recorded and the standard English designation, and whether a product satisfies a US requirement is for the civil surgeon.
Solar Hijri dates
Iranian documents are dated in the Solar Hijri calendar, whose year begins at the spring equinox, so 1402 spans parts of 2023 and 2024 and a conversion by subtracting a constant is wrong a substantial fraction of the time.
We convert each date individually and show both. In an immunisation record the interval between doses is what a civil surgeon assesses, so the dates have to be right.
Where the clinical record sits, and the national code
Iranian medical documents carry the national code, ten digits with a check digit, which does not change and is the cross-check across the file. Hospitals retain the chart and issue a discharge summary.
