One system holds everything
Costa Rica runs a single integrated public health system through the Caja Costarricense de Seguro Social, and it has built EDUS, a national electronic health record covering the whole population.
That means a Costa Rican can obtain a complete history — consultations, prescriptions, laboratory results and immunisations — from one source, which almost nowhere else on this list can offer. For an I-693 the EDUS immunisation report is the document, and most people do not know it exists.
The carné de vacunas, and the schedule
Childhood immunisation is also recorded on a carné de vacunas stamped at the EBAIS or clinic that gave each dose. Costa Rica gives BCG at birth, which is not a US vaccine and leaves a scar examiners ask about.

The cédula, and the EBAIS
Costa Rican medical documents carry the cédula number, assigned at birth registration and unchanged for life, which is the cross-check across the file.

Costa Rican physicians sign with their colegio de médicos registration, and we reproduce it. A substantial private sector serves medical tourism and issues records directly to the patient, frequently in English.
