The Certificado Esquema de Vacunación
Uruguay issues a Certificado Esquema de Vacunación — a certificate confirming the immunisation schedule is complete — which is required for school enrolment, for many public procedures and for employment in several sectors.
Because it is demanded so widely, Uruguayans generally have one and it is easy to reissue. For an I-693 it is the cleanest document, and it is better than the card because it is a formal certificate rather than a set of stamps.
The carné de salud, which is a different thing
Uruguay also requires a carné de salud — a periodic health examination certificate — for employment, and it is not an immunisation record.
The two documents are routinely confused because both are called a health card in conversation. We name which one you sent and what it certifies.
ASSE, mutualistas and where the record sits
Care is delivered through ASSE, the state provider, and through mutualistas — member-based private providers — under the national health fund. The clinical record sits with the provider.
So a Uruguayan history is split across providers if the person changed institution. We name the institution on each document.
The cédula, and the physician registration
Uruguayan medical documents carry the cédula de identidad, seven digits plus a check digit after a hyphen, which does not change and is the cross-check across the file.
