Buku Nikah or Akta Perkawinan, depending who married you
Indonesia issues two different marriage documents to two different populations. Muslim couples marry at the KUA, the office of religious affairs, and receive a Buku Nikah — a booklet with a stiff cover, closer to a passport than a certificate. Everyone else registers with Dukcapil, the civil registry, and receives an Akta Perkawinan, a sheet.
They are not the same document and they do not travel equally well: the civil registry akta is the one foreign institutions recognise most readily, and couples holding a Buku Nikah are sometimes asked for more. What your case needs is for your attorney to say; what we can do is translate either, in full.
One legal name, and the NIK
Many Indonesians have a single legal name with no family name at all, and US forms frequently record FNU or LNU in response — a convention for a missing field rather than a statement about the person. We reproduce the name exactly as given and say plainly that a single name is the complete legal name.

What USCIS requires
Under 8 CFR 103.2(b)(3), a foreign-language document must be accompanied by a complete English translation and the translator's certification that it is accurate. Every seal, stamp and marginal note is part of the document, so every one is translated.

That is the KUA booklet issued to Muslim couples and it is genuine. Some foreign institutions ask for more; that is a question for your attorney. We translate the booklet in full.
