Odpis skrócony and odpis zupełny are different documents
A Polish Urząd Stanu Cywilnego issues two kinds of copy and only one of them carries the history. The odpis skrócony — the abridged copy — gives the current state of the record. The odpis zupełny — the full copy — gives the entry and every annotation made to it since, including a divorce, an annulment or a correction.

Diacritics, and declined surnames
Polish uses ą, ć, ę, ł, ń, ó, ś, ź and ż, and stripping them changes words and names. We reproduce them, and where a passport or a US record spells a name without them we note the difference rather than silently choosing one.
Polish surnames also decline, and a wife's form can differ from her husband's by a syllable — Kowalski and Kowalska are the same family name. That reads as an inconsistency on a US form and is not one, so the translation makes the structure explicit. See our Polish page.
What USCIS requires
Under 8 CFR 103.2(b)(3), a foreign-language document must be accompanied by a complete English translation and the translator's certification that it is accurate. Every seal, stamp and marginal note is part of the document, so every one is translated.

Why do my wife's and my surnames differ?
Polish surnames decline by gender. Kowalski and Kowalska are the same name. We make that explicit so nobody reads it as a mismatch.
