Short copy or full copy — they are not the same
Polish civil records come from the Urząd Stanu Cywilnego, and it issues two versions of a birth record. The odpis skrócony is an abbreviated copy showing the main facts. The odpis zupełny is a full copy reproducing the entire register entry, including every annotation added later — a correction, an acknowledgement of paternity, a change of name.
Those annotations are frequently the reason an office asked for the record. If a full copy was requested and the short one is filed, it comes back. Check which one before ordering.
Diacritics, and citizenship by descent
Polish uses ą, ć, ę, ł, ń, ó, ś, ź and ż, and they are letters rather than decoration — Łódź is not Lodz. We keep the spelling as issued and match your passport.

Whichever the receiving office asked for. Only the full copy carries the marginal annotations, and those are often the point.
