A US record that is written in Spanish
Puerto Rico is part of the United States and its marriages are registered by the Registro Demográfico of the Department of Health. The certificate is a domestic US record — and it is written in Spanish, which is why a mainland office that cannot read it asks for a translation of a document it will then treat as domestic.

Two surnames, and what the 2009 law did not touch
Puerto Rican records carry two surnames and both are part of the legal name; dropping the second is how a person stops matching their own file.
Law 191 of 2009 invalidated Puerto Rican birth certificates issued before 1 July 2010, and people still arrive holding one. That invalidation does not extend to marriage certificates, so an older marriage certificate is not void for that reason — but if your file also needs a birth certificate, check that one.
What USCIS requires
Under 8 CFR 103.2(b)(3), a foreign-language document must be accompanied by a complete English translation and the translator's certification that it is accurate. Every seal, stamp and marginal note is part of the document, so every one is translated.
