The libreta de salud del niño
Paraguayan childhood immunisation is recorded in a libreta de salud del niño stamped at the health unit that gave each dose, under the MSPBS national programme.
Paraguay gives BCG at birth and, in defined departments, yellow fever. We reproduce every entry with its date, lot and unit.
MSPBS, IPS and the split record
Care runs through MSPBS Ministry of Health facilities, the IPS social security institute for insured workers, and a private sector, and each keeps its own record.

The cédula, and the reissued cards
Paraguayan medical documents carry the cédula de identidad civil number, issued by the police identification department, which stays with the person across a change of name.
Paraguay has reissued the card in several formats, so a file may hold very different-looking documents carrying the same number. Both are genuine.
Guaraní in the consultation
Paraguay is genuinely bilingual and most of the population speaks Guaraní, so a clinical record may note a consultation conducted in it, and Guaraní terms for symptoms and remedies appear in notes.
Guaraní names carry a nasal marker that changes the word when stripped, and registry clerks have historically hispanicised indigenous names. We reproduce each spelling exactly. See our Spanish page.
