There is no unified national record
Somalia has no functioning unified health record system. Childhood immunisation is delivered through Ministry of Health facilities, UNICEF and WHO campaigns, and a large NGO sector, and recorded on whatever card the provider issues.
So a Somali immunisation record may be an MCH card, an NGO clinic card, or nothing at all. We name the issuing body exactly and describe the document for what it is rather than presenting it as a national record.
Records created outside Somalia
A great many Somali medical records in US files were created in refugee camps in Kenya or Ethiopia, at UNHCR or NGO clinics, or by IOM during resettlement processing.
IOM resettlement health assessments in particular follow standards a US civil surgeon recognises and frequently include a documented immunisation series. Those are the most useful records in a Somali file and applicants often have them without realising.

Authorities in Somaliland operate their own health administration from Hargeisa, and documents name it. We name the issuing authority exactly as printed, without commentary on status.

Somali names run given name, father's name, grandfather's name with no inherited family name, so a US form demanding a surname produces a different answer each generation.
