The tarjeta de vacunas, and two systems
Panamanian childhood immunisation is recorded on a tarjeta de vacunas stamped at the facility that gave each dose, and care is delivered through two parallel systems — the Ministry of Health and the Caja de Seguro Social.
A family may have used both, so the immunisation history and the clinical record can be split across two networks that do not share files. We name the institution on each document.
BCG, and the schedule
Panama gives BCG at birth and, for travel to and from defined regions, yellow fever — neither routine in the United States.
We give the Spanish entry, the standard English designation and the date exactly as recorded. The card is what an I-693 needs.
The cédula, with its province prefix
Panamanian medical documents carry the cédula written as a province prefix, a tomo and a folio — and prefixes PE, E and N mark birth abroad to Panamanian parents, foreigners and naturalised citizens.
The prefix is part of the identifier and is not punctuation. We reproduce it in full.
The Canal Zone, and English-language records
People treated in the former Canal Zone before 1979, or at facilities serving it, may hold US-format records in English alongside Panamanian ones in Spanish.
