The NADRA certificate is the one that counts
A Pakistani birth is registered with the local union council, and NADRA issues the computerised certificate from that record. The two are not interchangeable for the purpose you probably have: visa and immigration processes require the computerised NADRA certificate, and a manual union council slip has no standing for them.
If what you hold is a handwritten or manually typed union council certificate, obtain the NADRA one before paying anyone to translate the one in your drawer. Documents for use abroad also commonly need attestation by the Ministry of Foreign Affairs, which is a page of the packet and is translated with the rest.
Urdu, English, and names
NADRA certificates are largely in English with Urdu alongside, and older union council documents are often wholly in Urdu. A partly-English document still needs the rest translated, and the translation covers the whole page rather than only the parts not already in English.

What USCIS requires
Under 8 CFR 103.2(b)(3), a foreign-language document must be accompanied by a complete English translation and the translator's certification that it is accurate. Every seal, stamp and marginal note is part of the document, so every one is translated.

A standard translation is for business or personal use where certification is not needed, such as CVs, client intake forms or marketing material, and is billed at the same $24.95 per page of up to 250 words.
