Names without surnames, and honorifics that are not names
Burmese names have no family name. A person's name is a sequence of syllables that is theirs alone, not shared with parents or siblings, which means a US form asking for a surname has nowhere correct to put anything — and it means two siblings' documents look unrelated.
Burmese names also carry honorifics that are not part of the name: U and Daw for older men and women, Ko and Ma for younger. A translation that folds "U" into the given name creates a name the person does not have. We reproduce the name as written and identify the honorific as an honorific.
Script, and the household list
Burmese is written in its own script, and documents commonly travel with a household registration list that establishes who lives together — which is frequently what an office needs to show a family relationship, rather than the birth certificate alone.

That is a question for the office or your attorney, not for a translator. We reproduce your name exactly as the document gives it and make the structure explicit, so nobody invents a surname on your behalf.
Is "U" part of my father's name?
