The acta de grado is a separate document
Colombian universities issue the diploma and, separately, the acta de grado — the formal record of the graduation ceremony, naming the degree, the date and the resolution under which it was conferred. Many institutions and evaluators ask for both, and an applicant who sends only the diploma gets asked for the other one weeks later.
Transcripts are issued as certificados de notas showing subjects, credits and marks, typically on a 0.0 to 5.0 scale with a pass around 3.0 — a scale that means nothing to a US reader without the key, which is why the key on the document is translated with it.
The diploma is the easy half
A degree certificate says you graduated. The transcript says what you studied, how you were assessed and how many hours it took, and it is the document an admissions office or a credential evaluator actually reads. It is also the harder document: a table of dozens of rows where every course title, mark, credit value and term has to survive in the same order and the same structure.
Grades are reproduced, never converted. Turning a foreign scale into a US grade point average is credential evaluation, not translation, and the agencies each require something different — WES will not accept a translation made by the applicant, while others will. What USCIS requires
Under 8 CFR 103.2(b)(3), a foreign-language document must be accompanied by a complete English translation and the translator's certification that it is accurate. Every seal, stamp and marginal note is part of the document, so every one is translated.
