A mayor or a notary can end a marriage in Peru
Under Ley 29227, Peru allows a marriage to be dissolved through a non-contentious procedure at a municipality or a notary, where both spouses agree and have been married at least two years. It runs in roughly two to three months, and what it produces is a municipal resolution (resolución de alcaldía) or a notarial instrument — not a court judgment.
A US officer expecting a court order may not recognise a mayor's resolution as a divorce at all. The translation names the instrument for what it is, and reproduces the resolution number and the issuing authority exactly. Contested divorces still go through the courts and produce a sentencia.

However the marriage ended, the dissolution is recorded in the civil registry and annotated on the marriage entry, and a marriage certificate issued afterwards carries the annotation. A certificate printed before it reads as a marriage still in force.

Do I need the judgment or the marriage certificate with the annotation?
Ask the office receiving it. The annotation proves the marriage ended; only the judgment or resolution carries the terms. Some filings want both and they are not interchangeable.
Is the date it became final translated?
Yes, and it is usually the date that matters. A decision and the date it became enforceable can be different days, and both are reproduced as the document gives them.
