The maternal and child health handbook
Myanmar childhood immunisation is recorded in a maternal and child health handbook and on EPI cards, administered through township health departments, rural health centres and midwives rather than hospitals.
So the record comes from a township office and is stamped by it. It is what an I-693 needs, and applicants routinely have nothing else.
Coverage, conflict and the gaps
Myanmar gives BCG and OPV early, and coverage has varied sharply by state and by period, with displacement and conflict interrupting delivery in several regions.

Names without surnames, and the dates
Burmese names are not divided into given and family names, and U, Daw, Ko, Ma and Maung are titles rather than name components. A US form demanding a last name splits a single name arbitrarily.
