How we know · policy checked on October 21, 2026
The Utah Division of Professional Licensing (DOPL) publishes no translation rule for physician applicants: Utah law requires graduates of medical schools outside the US to hold current ECFMG certification, and every applicant must have an FCVS packet released to Utah before applying.
ECFMG accepts certified translations from professional services such as eVerbary and does not let applicants translate their own documents.
We translate your diploma, transcripts and other records; ECFMG and FCVS verify them, and we do not order either.
Utah DOPL at a glance
Does it accept eVerbary? Yes
Utah requires foreign graduates to submit current ECFMG certification (Utah Code 58-67-302), and ECFMG accepts a word-for-word translation by a professional translation service (ECFMG translation rules), so a certified translation from a professional service such as eVerbary meets the rule that applies. Worth knowing about Utah DOPL
ECFMG by statute
Required of every graduate of a school outside the US.
FCVS first
The packet must reach Utah before you apply.
$235 application
Plus an optional $100 controlled substance license.
Utah DOPL requirements in detail
Translation
No translation rule in DOPL's physician application or pages (Physician and surgeon application, v20260324).
Evaluation
Graduates of medical schools outside the US submit current ECFMG certification (Utah Code 58-67-302, qualifications for licensure).
Who may translate (ECFMG)
A government official, a medical school official or a professional translation service; applicants may not translate their own documents (ECFMG: English translations).
FCVS
Required: you must have FSMB's email that your FCVS packet was released to Utah before submitting, unless you apply for a temporary license (Physician and surgeon application, v20260324).
Other items
AMA Profile or ABMS certification, the NPDB report you request, fingerprints, and $235 (Apply for a physician and surgeon license, application).
Without a Social Security number
An ITIN, A-number, or an unexpired passport with an intent-to-hire letter from a Utah employer (Physician and surgeon application, v20260324).
How to apply
Online, or on paper by mail or in person at the Heber M. Wells Building (Physician and surgeon application, v20260324).
ECFMG and FCVS carry the foreign file
Utah does not take medical credentials from you.
Its statute makes current ECFMG certification the proof of education for graduates of schools outside the United States (Utah Code 58-67-302, qualifications for licensure), and the application requires an FCVS packet released to Utah before you file (Physician and surgeon application, v20260324).
The DOPL FAQ tells applicants to contact FSMB, which notifies you once the report reaches the state (Physician and surgeon FAQ).
Translations are judged where ECFMG certifies your diploma and transcripts.
ECFMG wants an official, word-for-word translation, prepared from the original or a copy, signed, on letterhead identifying a translation service, with a certification statement, and it does not accept translations by applicants (ECFMG: English translations). Our certified translations are made to that standard.
Restricted associate physicians
The restricted associate physician and surgeon license uses the same list: FCVS packet released to Utah, NPDB report and fingerprints (Restricted associate physician and surgeon license). The translation work is the same as for a full license.

What an eVerbary translation includes
- A complete English translation of every page, stamps, seals and handwritten notes included, laid out like the original so each line can be matched.
- Grades, hours and credits exactly as recorded, with the grading scale the document prints. We never convert marks to a US GPA or equivalency.
- A signed certificate of translation accuracy on letterhead, with our contact details, delivered as a PDF you can upload or print.
- $24.95 per page of up to 250 words, certification included; 24 hours for one to three pages as standard. Regular clients who need it can get delivery in as little as 30 minutes, on request.
- Notarization added at checkout ($34.90 per order) when the office asks for a notarized translation.
Submitting the translation to Utah DOPL
Translations of your medical credentials go to ECFMG with the originals, following its instructions (ECFMG: English translations); ECFMG certification then reaches Utah with your FCVS packet.
If DOPL asks for any other document not in English during review, send the certified translation with it, by mail to P.O. Box 146741, Salt Lake City, or in person (Physician and surgeon application, v20260324).
What we do not do
eVerbary is a translation service, not a credential evaluation service or a licensing agency: we do not issue evaluations, US equivalencies or GPA conversions, we cannot send documents on behalf of the school or authority that issued them, and we do not file the license application.
For Utah that means we translate your documents for ECFMG and FCVS; we do not obtain ECFMG certification or the FCVS packet, and we cannot send records for your medical school.
How to order
- Scan or photograph every page of the document, front and back, including any stamps on the reverse.
- Upload it, choose the language and "certified", and see the price before paying.
- Receive the PDF, usually within 24 hours for one to three pages, and submit it with the copy of the original wherever the office asks for it.
