How we know · policy checked on October 21, 2026
The Wisconsin Medical Examining Board, through the Department of Safety and Professional Services (DSPS), publishes no translation rule on its physician licensing pages; foreign medical graduates upload their ECFMG certificate, and schools verify education directly or through FCVS.
ECFMG accepts certified translations from professional services such as eVerbary, so the translation is done for ECFMG. We translate your documents; we do not obtain your ECFMG certificate or FCVS profile.

DSPS asks foreign medical graduates for their ECFMG certificate (initial licensure), and ECFMG accepts a word-for-word translation by a professional translation service (ECFMG translation rules). Policy checked on October 21, 2026.

Wisconsin MEB requirements in detail
Translation
No translation rule on DSPS's physician licensing pages (Physician MD/DO initial licensure).
Evaluation
Foreign medical graduates upload a copy of their ECFMG certificate; it is also accepted through FCVS (Physician MD/DO initial licensure).
Who may translate (ECFMG)
A government official, a medical school official or a professional translation service; applicants may not translate their own documents (ECFMG: English translations).
Medical education
You send Form 2164 to your medical school, which verifies your education to DSPS, or you use FCVS (Physician MD/DO initial licensure).
FCVS
Optional. With an FCVS profile you may not need Forms 2164 and 2165, exam score reports, the FSMB and NPDB reports or the ECFMG upload (Physician MD/DO initial licensure).
Provisional license
For applicants with a medical degree and training outside the US; foreign graduates upload their ECFMG certification here too (Physician MD/DO provisional licensure).
Why the translation is ECFMG's business in Wisconsin
DSPS does not take your medical records from you. Your school verifies your education on Form 2164, or FCVS does, and as a foreign graduate you upload your ECFMG certificate (Physician MD/DO initial licensure). The point where a translation is judged is ECFMG certification.
ECFMG asks for an official English translation of any document not in English, word for word, prepared from the original or a copy, on letterhead identifying a translation service, with a certification statement; it does not accept translations by applicants (ECFMG: English translations). Our certified translations are made to that standard.
The provisional license for internationally trained physicians
Wisconsin's provisional license is for applicants who received a medical degree and trained in a program outside the United States with requirements substantially similar to full licensure (Physician MD/DO provisional licensure).
It follows the same pattern: Form 2164 from the school or FCVS, and the ECFMG certificate uploaded. If DSPS asks you for an additional foreign document during review, send a certified translation with it.

What an eVerbary translation includes
- A complete English translation of every page, stamps, seals and handwritten notes included, laid out like the original so each line can be matched.
- Grades, hours and credits exactly as recorded, with the grading scale the document prints. We never convert marks to a US GPA or equivalency.
- A signed certificate of translation accuracy on letterhead, with our contact details, delivered as a PDF you can upload or print.
- $24.95 per page of up to 250 words, certification included; 24 hours for one to three pages as standard. Regular clients who need it can get delivery in as little as 30 minutes, on request.
- Notarization added at checkout ($34.90 per order) when the office asks for a notarized translation.
Submitting the translation to Wisconsin MEB
Translations of your medical credentials go to ECFMG with the originals, following ECFMG's instructions (ECFMG: English translations); ECFMG then issues the certificate you upload to DSPS (Physician MD/DO initial licensure).
If DSPS asks for any other document that is not in English, upload the certified translation together with the original in your application documents area.
What we do not do
eVerbary is a translation service, not a credential evaluation service or a licensing agency: we do not issue evaluations, US equivalencies or GPA conversions, we cannot send documents on behalf of the school or authority that issued them, and we do not file the license application.
For Wisconsin that means we translate your records for ECFMG and DSPS; ECFMG certifies them, and your school or FCVS verifies them.
How to order
- Scan or photograph every page of the document, front and back, including any stamps on the reverse.
- Upload it, choose the language and "certified", and see the price before paying.
- Receive the PDF, usually within 24 hours for one to three pages, and submit it with the copy of the original wherever the office asks for it.
