What ECFMG asks for, which is not what most agencies ask for
A physician educated outside the United States runs into ECFMG before anything else, and its translation rules are unusually specific. As published in September 2026:
- The translation must be word-for-word. An abstract or summary translation is not acceptable.
- It must be prepared from the original document or a photocopy of it — not from a transcribed version.
- The original-language document and the translation must be submitted together. Neither is accepted alone.
- It must carry a certification statement and the signature and title of the person or service that prepared it, with a seal where there is one.
- It must appear on letterhead, and if a private company prepared it the letterhead must identify the company as a translation service.
- Applicants may not translate their own documents.

Beyond ECFMG: state boards and hospital credentialing
The medical diploma and transcripts are only the start. State medical boards, residency programmes and hospital credentialing offices ask for their own set: the licence to practise issued in your home country, certificates of good standing or of no disciplinary action, internship and residency certificates, specialty board certificates, and letters of verification from the institutions where you trained.
Each body sets its own rules, so check the one you are applying to. What is constant is that every non-English document needs a complete, literal English translation with a signed certification attached.

Certified vs. standard translation
A certified translation is a word-for-word translation accompanied by a signed statement attesting to its accuracy and completeness. It is what official bodies require: USCIS, courts, universities and government agencies. Certified translations are $24.95 per page, where a page is up to 250 words.
A standard translation is for business or personal use where certification is not needed, such as CVs, client intake forms or marketing material, and is billed at $0.10 per word.
Frequently asked questions
Can I translate my own medical diploma for ECFMG?
No. ECFMG states that applicants are not permitted to translate their own documents, and that a translation must come from a government official, a medical school official or a professional translation service.
Does the original-language document have to be sent too?
Yes. ECFMG will not accept a translation without the original-language document, or an original-language document without a conforming translation.
Do you translate the certificate of good standing from my home country board?
Yes, along with specialty certificates, residency certificates and verification letters. Send them together so names and dates stay consistent.
