The state language examination
Slovak proficiency is examined by university language centres and, for defined purposes, by a state language examination, and knowledge of Slovak is required for naturalisation.
So a Slovak language certificate is frequently a legal precondition rather than an academic credential, and the certificate names the examining body and the level. We reproduce both.
Slovak and Czech
Slovak and Czech are mutually intelligible for adults who grew up with both, and Slovak law and practice have historically treated Czech as understood without translation in official contexts.
So a Czech certificate is in practice evidence about Slovak to a degree, and institutions vary in how they treat it. We name the language the certificate states without asserting what it evidences about the other.
The Hungarian minority
Several hundred thousand Slovak citizens are first-language Hungarian speakers, and Slovak may be a second language for them, examined in school and certified accordingly.
So a Slovak certificate held by a Slovak citizen may attest a second language. We do not infer a first language from nationality.
Diacritics, and the -ová
Slovak diacritics are reproduced from the printed page, and a candidate's surname may carry the feminine -ová ending or the form without it, which Slovak law permits.
