ZAGS certificates, Soviet and modern
Russian births are registered by the civil registry office, the ЗАГС (ZAGS), which issues the свидетельство о рождении. Certificates issued by the Soviet ZAGS before 1991 remain valid and turn up constantly; they are a different size and layout from the modern Russian form, and some are handwritten.
A certificate issued in the Soviet period may name a republic that is now an independent country. That is not an irregularity — it is where the registration happened — and the translation says what the document says rather than updating it to today's map.
The patronymic is not a middle name
A Russian name is given name, patronymic and surname, and the patronymic is built from the father's given name with an ending that marks sex: Ivanovich for a son, Ivanovna for a daughter. It is not a second given name, and it is not optional — it appears on every official document.
Surnames are gendered too, so a husband and wife appear under forms of the same surname that differ by one letter. Both patterns read as inconsistencies on a US form and are neither. What USCIS requires
The requirement is the same for every country: under 8 CFR 103.2(b)(3), a foreign-language document must be accompanied by a complete English translation and the translator's certification that it is accurate.
